    Job Location:  550 West 700 South, SLC, UT  84101
     
    Schedule:  40 - hrs; M-F 8:00 - 5:00
     
    Pay:  Competitive - commensurate with experience...PLUS:
    • 5% increase in pay with demonstrated, bilingual English/Spanish fluency
    • Sign on bonus, student loan repayment, retention bonuses negotiable
    • If moving from another state relocation package available

    Responsibilities

    • Mental Health Medication Management
    • Monitor client medication compliance
    • Coordination of care with internal and community partners 
    • Coordinate with a multidisciplinary team of professionals
    • Work with Psych team members to be a resource for the nursing home
    • Follows all organization policies and procedures
    • Meets required productivity standards if applicable and other organizational expectations
    • Document services and coordination provided using our EHR system

    Qualifications

    • Must hold current APRN license in good standing with the State of Utah
    • Self-motivated
    • Eager to actively contribute to improving processes and client care
    • Communicate effectively verbally and in writing
    • Establish and maintain effective inter/intra organization relationships
    • Work as member of multi-disciplinary team
