    We are seeking a compassionate APRN for full time employment at our Specialized Rehabilitation Services program. Valley Behavioral Health's Specialized Rehabilitation Services (SRS) is a professional, community-based, therapeutic services organization with team members operating programs that provide mental health medication management and services for adults 20 years of age and older residing in extended care facilities (i.e. nursing homes, assisted living, etc.) who are experiencing  mental health disorders. We have a well trained and caring staff of therapists, psychiatrists, case managers, and social workers to support you.

    Locations: Salt Lake City

     

    Schedule: M – F, days (no on-call or weekends!)

     

    Pay: Commensurate with experience

    Responsibilities

    • Mental Health Medication Management
    • Monitor client medication compliance
    • Coordination of care with internal and community partners 
    • Coordinate with a multidisciplinary team of professionals
    • Work with Psych team members to be a resource for the nursing home
    • Follows all organization policies and procedures
    • Meets required productivity standards if applicable and other organizational expectations
    • Document services and coordination provided using our EHR system

    Qualifications

    • Must hold current APRN license in good standing with the State of Utah
    • Self-motivated
    • Eager to actively contribute to improving processes and client care
    • Communicate effectively verbally and in writing
    • Establish and maintain effective inter/intra organization relationships
    • Work as member of multi-disciplinary team
