    Schedule:  Part time, 32 hours per week, Monday-Friday

     

    Location:  Tooele - 27 S Main Street, 84074

     

     Benefits: Since this position is over 30 hours per week you would be eligible for medical benefits through Valley Behavioral Health.

     

    Responsibilities

    • Community mapping and knowledge of resources
    • Work with individuals, families and community partners to provide wrap around care
    • Crisis intervention and outreach as needed
    • Provide positive role modeling
    • Respond to situations that may arise in a professional and calm way
    • General Office Case Management for walk-in clients
    • Maintain accurate and timely paperwork
    • Required training and documentation through Utah Family  Coalition/Family Resource Facilitator Program
    • Other duties as assigned

    Qualifications

    • Must be 21 years of age with good boundaries and the maturity necessary to model appropriate behavior and create a positive environment
    • Must have 2 years education or work experience in the Human Service Field
    • Experience working with children, youth and families with mental health a real plus!
    • Must be able to work effectively as a member of a multi-disciplinary team
    • Optimistic individuals with excellent teamwork and customer service focus
    • Basic computer skills and efficiency with MS Office and ability to learn new applications quickly
    • Personal vehicle required to travel to trainings and meeting with clients
    • Work as an effective team member in keeping client(s) as highest focus
    • 5% wage increase for proven bilingual English/Spanish proficiency
