We are seeking a Clinical Director to work with our Assertive Outreach Teams including our AOT Hospital Team, Jail Diversion Outreach Team, Homeless Outreach Teams, IWRAP (children’s outreach) and Tooele Assertive Outreach Team.

Schedule:        Monday-Friday 8:00am-5:00pm

Pay:                 Negotiable with experience

Responsibilities

Oversee all VBH assertive outreach teams to ensure the teams are:

      • Teaching client independent living skills in order to reduce and prevent return to hospitalization. 
      • Coordinate with community partner organizations to link clients with needed resources.
      • Assist clients to gain access to needed medical, social, educational, and other services and to ensure that services are coordinated among all agencies and providers involved.
      • Develop therapeutic relationship with clients and their families
      • Crisis intervention
      • Based on Client Care Plan:    
  • Milieu supervision
  • Monitor progress
  • Teach skills
  • Link to community resources
  • Document and bill accordingly
      • Ensure continuity of care
      • Monitor hospitalization data and KPI data for the teams
      • Other duties as assigned
