Overview

 
We are looking for a case manager  that works hard and is willing to teach skills and provide personal services to those high acuity clients currently housed at Safehaven, our temporary housing for our homeless population.   
 
Safe Haven II has 24 permanent housing units for homeless individuals challenged by a history of chronic hoelessness, mental health and substance use issues.  They are assisted with apartment living/home maintenance, medication management, benefit, management, skills development, socialization and peer support services.
 
Location:  550 West 700 South, SLC, UT  84101 
 
Pay:  Commensurate with experience
 
Schedule: 3:00 pm-11:00 pm

Responsibilities

  • Teach client independent living skills in order to reduce and prevent return to homelessness
  • Coordinate with community partner organizations to link clients with needed resources
  • Assist clients to gain access to needed medical, social, educational, and other services and to ensure that services are coordinated among all agencies and providers involved
  • Develop therapeutic relationship with clients and their families
  • Crisis intervention
  • Based on Client Care Plan:    
    • Milieu supervision
    • Monitor progress
    • Teach skills
    • Link to community resources
    • Document and bill accordingly
  • Ensure continuity of care
  • May distribute medications after obtaining proper certification
  • May transport clients in personal vehicle
  • Other duties as assigned

Qualifications

  • Must be able to pass a background check.  
  • High School Diploma/GED or higher
  • 2+ years’ experience in human services
  • State issued Case Manager Certificate or ability to obtain certification within 30 days of employment, preferred.
    • **Valley Behavioral Health will assist you in obtaining this certification**
  • Proficiency in Microsoft Office with the ability to learn new software programs quickly
  • Meet drivers qualification requirements which includes a clean driving record with no more than 3 traffic tickets within 3 years
  • Pro-active team focus
  • Knowledge of appropriate community resources crucial to assisting our clients to live independently
