Overview

We are searching for a Hospital Liason on our Assertive Outreach Team to work with the hospital or inpatient facility and our clients or potential clients of Valley Behavioral Health to provide continuity of care and support as clients transition back to the community.  Ideal candidates will have at least 2 years of experience in the mental health field and/or a related degree.  Case management experience in the mental health field would be an asset.  This is a position where you will have a tremendous impact on clients who are in a difficult and critical transition period.

 

The Assertive Outreach Team Program (AOT) is a multidisciplinary, 24/7, intensive, short-term, wrap around program serving clients with severe and persistent mental illness and substance abuse. AOT is designed to provide comprehensive, community based treatment to assist individuals in reintegrating back into the community through stabilization and successful transition to the needed level of care. The AOT team focus is to reduce the use of the “revolving door” hospitalizations, promote growth and recovery, and enhance the quality of personal, family, and community life. Length of services will be linked directly to medical and clinical necessity and client readiness to step down to VBH Outpatient Programs.

Schedule: M-F, 8-5

           

Location:  Salt Lake City

 

Pay:  Commensurate with experience

Responsibilities

  • Hospital inreach for all adult and adolescent clients of VBH to provide continuity of care and support as clients transition back to the community.
  • Hospital inreach and engagement for potential new clients of VBH being referred by inpatient providers.

Qualifications

  • At least 2 years of education or work experience in the Mental Health Field.
  • Preferred Bachelors degree in social work or a related field.
  • Case management experience preferred.
  • Self motivated and team focused. Will be part of a large multidisciplinary team!
