We are seeking a full-time, dedicated, resourceful, and solution-focused Case Manager for our Summit Program. The Summit County program is part Valley Behavioral Health’s supported outpatient program for Adults with a variety of substance abuse and mental health issues. This dynamic, interdisciplinary team utilizes a specialized, recovery-based approach to provide a variety of wrap around services.

Location: 1753 Sidewinder Dr. Park City, UT 84060
Schedule:  Monday - Friday 8:00 - 5:00 (hours may vary)  
Pay:  Commensurate with experience

  • Work with high acuity and all aged clients that may sometimes be in crisis
  • Meeting with the incarcerated population of Summit County
  • Help individuals with substance abuse and legal involvement to reduce recidivism
  • Assist and gain access to needed medical, social, educational and other services
  • Coordinate services between all agencies and providers involved
  • Develop therapeutic relationship with clients and their families
  • Crisis intervention
  • Ensure continuity of care
  • May transport clients
  • Work as an effective team member keeping client as highest focus
  • Other duties as assigned




  • 2+ years experience in the behavioral health field
  • State issued Case Manager Certificate or ability to obtain certification within 30 days of employment
  • Experience and/or willingness to work with incarcerated population
  • Ability to document services in a timely manner
  • Excellent computer skills and efficiency with MS Office and ability to learn new applications easily
  • Meet driver qualifications required if applicable to essential job functions
  • Bilingual is a plus but not required
