Overview

We are looking for a person to track high risk clients on funding and to help with their medication management.  You will track doctor’s appointments with our prescriber and help the nurses with tasks.  This person will also act as a case manager.

 

JDOT is an ACT-like team, providing services in the community as well as in the office.  We have a smaller caseload with this multi-disciplinary team.  Services are highly individualized, flexible and staff are available 24/7. JDOT’s efforts have contributed to a 66% reduction in jail bookings and a 75% reduction in length of jail stay.

Locations: 1020 S. Main St., Suite 100, Salt Lake City, UT

 

Schedule: M – F, 8 AM – 4 PM

 

Pay: Commensurate with experience

Responsibilities

  • Outreach to client’s homes or in the community as needed
  • Coordination of care within Valley Behavioral Health and with outside providers
  • Administer medication
  • Responds to crisis situations
  • Follows all organization policies and procedures
  • Meets required productivity standards if applicable and other organizational expectations
  • Document services and coordination provided using our Online Medical Record
  • Other duties as assigned

Qualifications

  • Must obtain Adult Mental Health Case Manager certificate within 30 days of employment
  • Assess client needs without judgment related to own theoretical bias
  • Communicate effectively verbally and in writing
  • Establish and maintain effective inter/intra organization relationships
  • Work as a member of a multi-disciplinary team
  • Meet driver’s qualification requirements which includes a clean driving record with no more than 3 traffic tickets within the past 3 years
