Location: Tooele Adult Program 100 South 1000 Main Street Tooele, UT 84074
 
Pay: Commensurate with Experience
 
Hours: Monday - Friday 8:00am-5:00pm with some flexibility

Responsibilities

  • Assist clients to gain access to needed medical, social, educational, and other services and to ensure that services are coordinated among all agencies and providers involved.
  • Develop therapeutic relationship with clients and their families
  • Crisis intervention
  • Based on Client Care Plan
    • Milieu supervision
    • Monitor progress
    • Teach skills
    • Link to community resources
    • Document and bill accordingly
  • Ensure continuity of care
  • May distribute medications after obtaining proper certification
  • May transport clients
  • Work as an effective team member keeping client as highest focus
  • Other duties as assigned

Qualifications

  • 2+ years’ experience in related field (required)
  • State issued Case Manager Certificate or ability to obtain certification within 30 days of employment
  • Excellent computer skills and efficiency with MS Office and ability to learn new applications easily
  • Meet driver qualifications required if applicable to essential job functions
