The Wellness and Lifestyle Coach will be essential in identifying the high-risk clients and collaboratively creating care plans to help improve their overall mental and physical health. The idea is to have someone dedicated to personally assist our patients with identifying their health goals, create an achievable action plan and work with them through every step of the process!

Location: 4460 S. Highland Drive, Suite 200

Hours: M-F; 40 hours/week

Pay: Commensurate with Experience

Primary Responsibilities:

  • Identify high-risk individuals and create a care plan specifically taylored for each patient to address their areas for improvement (based on their goals)
  • Uses advanced clinical skills, good communication, experience and all available resources to make decisions regarding the medical and behavioral needs of each patient, in order to provide cost-effective, quality outcomes
  • Leading various initiatives one on one with the patient such as: excercise and nutrition classes, crafting health promotion programs, leading stress management activities, (among others you create)
  • Utilize interactive software and phone applications to communicate with patients to frequently track progress and improvment on agreed upon goals and implementation methods.
  • Collaborates with providers to determine acuity of mental and behavioral health concerns and refer enrollees to appropriate community resources
  • Works closely with the Family Nurse Practitioner within the Clinic in order to collaborate for best practices
  • Closely monitor outcomes and report on progress and patients' behavior modification

Qualifications

  • Masters (preferred) or Bachelors Degree in Health Education and Promotion, Registered Dietitian Nutritionist (RDN) or Population Health or a related field
  • A strong passion for helping to encourage and motivate others in order to help them achieve their personal goals...and the tenacity to remain optimistic with those who are more resistant to change.
  • Some experience in behavioral health or a medical facility is preferred
  • Must have excellent communication skills and the ability to effectively relate to clients and coworkers
  • Experience and an interest in working with ages 0-100
  • Able to maintain confidential information in a professional manner
  • Proficient in Microsoft Office Suite and able to learn new EHR systems quickly
