We are seeking a full-time, dependable, resourceful, dedicated Case Manager for our JDOT (Jail Diversion Outreach Team) Program. This team serves those who are experiencing homelessness that are at risk of hospitalization and/or incarceration due to mental illness and substance use disorders. Link individuals to the lifesaving resources and supportive housing.
Location:  1020 South Main Street SLC, UT 84101
Pay:  Commensurate with experience
Schedule:  Monday-Friday 8am-4pm

  • Provide direct case management services: Including assessment, goal setting, problem solving, service planning and support
  • Jail in-reach to meet with inmates in the Salt Lake County Jail
  • Coordinate with community partner organizations to link clients with needed resources
  • Assist clients to gain access to needed medical, social, educational, and other services and to ensure that services are coordinated among all agencies and providers involved
  • Develop therapeutic relationship with clients and their families
  • Crisis intervention
  • Ensure continuity of care
  • May transport clients in personal vehicle
  • Other duties as assigned


  • Must be able to pass a background check with Valley and Salt Lake County Jail.  
  • High School Diploma/GED or higher
  • 2+ years’ experience in Mental Health and Substance abuse treatment
  • State issued Case Manager Certificate or ability to obtain certification within 30 days of employment, preferred.
  • **Valley Behavioral Health will assist you in obtaining this certification**
  • Valid Drivers license
  • Proficiency in Microsoft Office with the ability to learn new software programs quickly
  • Meet drivers qualification requirements which includes a clean driving record with no more than 3 traffic tickets within 3 years.
