    We are seeking a dedicated, flexible, and resourceful Case Manager for our Assertive Outreach Team (AOT). The Assertive Outreach Team is the home unit for Valley Behavioral Health’s highest acuity adult clients suffering from severe and persistent mental illnesses as well as numerous other co-occurring and psychosocial conditions. We are community based serving our community’s highest need clients in an attempt to decrease hospitalizations and improve quality of life and stability.

    Pay: Earn from 29-33k per year including pay for perfomance! 



    Location:  Salt Lake County [mobile/outreach]


    Primary offices: 1020 South Main Street Salt Lake City, UT 84101 



    Schedule:  Monday - Friday; 8:00am - 5:00pm (with some flexibility due to crisis calls)

      • Provide case management interventions to high acuity, SPMI clients
      • Work with a multidisciplinary team of case managers, therapists and medical staff
      • Coordinate in conjunction with other Valley units as well as community providers and organizations for best client outcomes
      • Crisis intervention and de-escalation as required


      • Bachelors Degree or related experience
      • Must demonstrate compassion, flexibility and dependability
      • Must have ability to pass a background check
