We are seeking a dedicated, flexible, and resourceful Case Manager for our Assertive Outreach Team (AOT). The Assertive Outreach Team is one of Valley Behavioral Health’s crisis outreach programs for adults who may be experiencing severe/persistent mental health issues (SPMI). This interdisciplinary team implements several components of wrap-around community-based treatment modalities to provide strategies that promote the highest quality of life possible as determined by the individual. AOT collaborates with clients to create a personalized plan to better serve those who may have previously struggled to engage in more clinical settings.

Location:  Salt Lake County [mobile/outreach]

 

Primary offices: 1020 South Main Street Salt Lake City, UT 84101 

 

Pay:  $12.00 – $14.00/hour

 

Schedule:  Monday - Friday; 8:00am - 5:00pm (with some flexibility due to crisis calls)

Responsibilities

  • Work with identified SPMI clients to assess and monitor functioning in the least-restrictive setting possible in order to meets the needs of the individual served
  • Coordinate with community partner organizations, including continuity of care referrals
  • Link clients with needed resources to promote consistent and effective services / supports in different settings and circumstances
  • Develop therapeutic relationship with clients and their families
  • Crisis intervention and WRAP-informed planning
  • Manage multiple high-acuity clients
  • Maintain accurate records
  • Transport clients in personal vehicle to and from medical and psychiatric appointments
  • Help clients remain organized and maintain their benefits and housing
  • Other duties as assigned

Qualifications

  • Bachelors degree in Social Services or Behavioral Health or related filed 
  • 2+ year's experience in human services
  • State issued Case Manager Certificate or ability to obtain certification within 30 days of employment, preferred.
    • **Valley Behavioral Health will assist you in obtaining this certification**
  • Proficiency in Microsoft Office with the ability to learn new software programs quickly
  • Meet driver's qualification requirements which includes a clean driving record with no more than 3 traffic tickets within the past 3 years.
