FlexCare provides medical case management for nursing home eligible Medicaid clients. We help people move out of nursing homes and maintain independence in the community by identifying resources and coordinating services to meet their individual needs and preferences. RN and SSW teams then meet with the clients in the community and monitor them to make sure their needs are met. We coordinate with medical doctors, mental health and substance use providers, other service providers, families, Medicaid, and Medicare to help people have the highest possible quality of life.
