We are seeking a dependable, resourceful, dedicated Case Manager for our Storefront Team. This team serves those who are experiencing homelessness that are at risk of hospitalization and/or incarceration due to mental illness and substance use disorders. Link individuals to the lifesaving resources and supportive housing.
 
Location:  550 West 700 South, Salt Lake City, UT  84101 
 
Pay:  Commensurate with experience
 
Schedule:  Monday - Friday 8:00am - 5:00pm (Must have Flexibility)

Storefront is a non-traditional outpatient facility that provides services to SPMI Homeless individuals who may also have another disorder such as substance use. This acts as an entry point into treatment for those who are reluctant to try traditional mental health options. 

Responsibilities

  • Teach client independent living skills in order to reduce and prevent return to homelessness . 
  • Coordinate with community partner organizations to link clients with needed resources.
  • Assist clients to gain access to needed medical, social, educational, and other services and to ensure that services are coordinated among all agencies and providers involved.
  • Develop therapeutic relationship with clients and their families
  • Crisis intervention
  • Based on Client Care Plan:    
    • Milieu supervision
    • Monitor progress
    • Teach skills
    • Link to community resources
    • Document and bill accordingly
  • Ensure continuity of care
  • May distribute medications after obtaining proper certification
  • May transport clients in personal vehicle
  • Other duties as assigned

Qualifications

  • Must be able to pass an FBI background check.  
  • High School Diploma/GED or higher
  • 2+ years’ experience in human services
  • State issued Case Manager Certificate or ability to obtain certification within 30 days of employment, preferred.
    • **Valley Behavioral Health will assist you in obtaining this certification**
  • Proficiency in Microsoft Office with the ability to learn new software programs quickly
  • Meet drivers qualification requirements which includes a clean driving record with no more than 3 traffic tickets within 3 years.

  • Knowledge of appropriate community resources crucial to assisting our clients to live independently
